Shorter links and QR codes, and a few phone line fixes

Staff can now shorten any URL and generate a QR code for it from the Insight app's QR tool, built on a new short-link system (links.nsemm.org.uk). On the phone line, a caller who hears a website link read out can now press 1 to get it as a text message instead. We also fixed a couple of small issues with the phone line: the internal bypass code no longer triggers an irrelevant "have you called us before" question, a repeated-word wording glitch was fixed, and the escape-code confirmation tone was turned down after being reported as too loud.
